Professional Referral Form
Some clients feel more supported when their worker helps them make the first step. If you are an allied health professional, social worker or community worker, you can use this referral pathway so we can contact your client directly and guide them into their first session.
A referral is not required for AOD peer support. Clients can contact us themselves at any time. This option simply creates a smoother handover for those who need it.
How the referral works:
When you submit the form, we will:
Contact the client using their preferred method.
Confirm consent, explain how sessions run, and answer any questions.
Organise their initial assessment.
You will be notified if we cannot reach the client after reasonable attempts.
Before you refer:
Please make sure your client:
Understands what Your Recovery offers.
Has agreed to be contacted.
Feels comfortable with a peer-led, non-clinical support approach.
