Professional Referral Form
Most clients access Your Recovery by booking their own Initial Assessment through our website and paying online.
For organisations wishing to fund support for a client, we also offer a referral and invoicing pathway to reduce administrative burden and simplify access to support.
Organisation-Funded Referrals:
If your organisation would like to fund sessions for a client, you can submit a referral through the form below.
Once received, we will:
• Contact the client using their preferred contact method
• Explain how sessions work and answer any questions
• Arrange their Initial Assessment
• Provide invoicing directly to your organisation
You will be notified if we are unable to contact the client after reasonable attempts.
Package Option for Funded Referrals:
To simplify administration, organisations can choose a prepaid package which includes:
• 1 × 90-minute Initial Assessment (50% discounted)
• 5 × 60-minute Ongoing Sessions (10% discounted)
Total package cost: $519.75 inc GST
This package can be invoiced upfront to the referring organisation, reducing the need for individual session invoices.
